I, as the data subject, hereby give my express consent to the company Profinit EU, s.r.o., having its registered office at Tychonova 270/2, Praha 6, Company Number 04434081, entered in the Commercial Register maintained at the Metropolitan/Regional Court in Prague, file 247646 (hereinafter referred to as the “Controller”), to the processing of personal data transferred to the Controller, under the conditions specified hereunder: the personal data which shall be processed: first name and surname, age, postal address, e-mail address, telephone contact, information about level of education attained, employment references and experience or skills, data of birth and, where appropriate, other information according to the CV provided or which the company finds in relation to me from publicly-available networks, i.e. I consent to their storage on data carrier, treatment, searching within them, sorting and combining for the purpose of sending the employment offers of the Controller. The period of personal data processing is ten years following the giving of consent. Personal data processing is and shall be undertaken by the Controller, or where appropriate a processor with which the Controller enters into a special contract on personal data processing, for the specified purpose. The data subject declares that he/she has been duly informed by the Controller of the processing and protection of personal data*, that the personal data specified above are accurate and truthful and are voluntarily provided to the Controller.
* Information provided to the data subject – The Controller hereby, in accordance with the provisions of Article 13 of REGULATION (EU) 2016/679 OF THE EUROPEAN PARLIAMENT AND OF THE COUNCIL of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (General Data Protection Regulation), informs the data subject that: (i) the personal data of the data subject shall be processed pursuant to his/her freely-given consent, under the conditions specified above; (ii) the reason for providing the personal data of the data subject is the interest which the data subject has in being sent employment offers from the Controller and participating in recruitment selection procedure for the employment positions offered by the Controller; (iii) when processing the personal data of the data subject, there shall be no automated decision-making or profiling (automated decision-making might occur regarding the rejection of an employment application if the data subject is on record at the Controller as an unsuccessful applicant); (iv) the Controller does not intend to transfer the personal data of the data subject to a third country, international organisation or other person other than the third parties specified above; (v) the data subject has, at any time, the right to withdraw his/her consent to personal data processing, the right to demand of the Controller access to his/her personal data, rectify them or erase them or restrict processing and to lodge an objection to processing, the right to the portability of such data to another controller and the right to lodge a complaint with Úřad pro ochranu osobních údajů (Office for Personal Data Protection) should he/she believe that the Controller is proceeding contrary to the Regulation when processing personal data.
